(Batesville, Ark) – White River Health (WRH) recently partnered with ChartSpan, a Chronic Care Management (CCM) solution provider, to offer CCM services to patients in North Central Arkansas. Qualifying Medicare patients with two or more chronic medical conditions can enroll in the program to receive personalized care plans and additional support from WRH healthcare providers.

ChartSpan offers qualifying patients access to a variety of services outside of their routine medical appointments. WRH providers and ChartSpan’s care coordinators work with patients to create a personalized care plan to help patients achieve their health goals. Qualifying patients also have access to a 24/7 nurse line, educational resources, Social Determinant of Health support, and assistance with appointments and refills. Participation in CCM can lead to a decrease in unintended hospital readmissions and lower overall health costs for patients.
