Community Health Needs Assessment

Every three years White River Health (WRHS) conducts a Community Health Needs Assessment (CHNA) to identify the health needs of the communities served and identify initiatives to those needs.  A part of the process is obtaining input from residents of the communities we serve. We want your input.  The survey takes just a few minutes to complete and no identifying personal information will be collected.  The survey is confidential and voluntary.
